Abstract MicroCog test results and supplemental neuropsychological test findings are reported concerning a male health professional who had abused nitrous oxide (N2O) on a regular basis for more than 20 years. At the time of the examination, the health professional had abstained from N2O use for 12 months. Test findings indicate that the patient had suffered a general decrement in cognitive functioning. Spatial functioning and reaction times were better preserved than attention, mental control, verbal and arithmetic reasoning, and verbal memory. His verbal memory loss was particularly severe, but abstract reasoning and practical social judgment remained intact. Marked visual memory and construction deficits were identified with supplemental clinical tests. Since 12 months of abstinence had elapsed prior to the examination, it may be reasonably assumed that cognitive recovery was nearly complete and the findings reported are stable. Key Words: chronic substance abuse, nitrous oxide abuse, cognitive deficits, neuropsychological deficits Nitrous oxide ([N.sub.2]O) abuse is rarely seen in the general practice of clinical and forensic psychology. Also, little has been reported on the cognitive consequences of chronic [N.sub.2]O abuse. For this reason, we offer the results of a neuropsychological evaluation completed in a VA Medical Centers Outpatient Behavioral Medicine Clinic on a chronic abuser of [N.sub.2]O.

The use of hypnosis in sports counseling and athletic training is modestly represented in the literature. However, almost all of the existing literature on sports hypnosis focuses on hypnosis for performance enhancement or aspects related to performance enhancement, such as concentration and stress reduction (Braybrooke, 1988; Collins, Doherty, Talbot, 1993; Heyman, 1987; Krenz, 1984; Liggett, 2000; Morgan, 1993; Robazza Bortoli, 1995; Stanton, 1994; Tafti, Besset, Billiard, 1992; Wojcikiewicz Orlick, 1987). This article will examine themes reflected in the literature on sports hypnosis and will explore uses of Heart-Centered Hypnotherapy (HCH) with athletes as demonstrated through case examples. Several authors provide general overviews of the uses of hypnosis in sports (Liggett, 2000; Morgan, 1993; Taylor, Horevitz, Balague, 1993). However, the majority of available literature on sports hypnosis focuses on using hypnosis for performance enhancement. In addition, many of the articles that do not focus on performance enhancement per se, describe characteristics which have a direct effect on performance. For example, this literature review found that the second most common use for hypnosis in sports was anxiety or stress reduction (Howard Reardon, 1986; Krenz, 1984; Naruse, 1965; Onestak, 1991; Wojcikiewicz Orlick, 1987). Another common theme in sports hypnosis literature (also connected to performance enhancement) is the relationship of hypnosis to mental and psychological training (Krenz, 1984; Onestak, 1991). In addition, hypnosis has been used for other aspects of performance enhancement, including strength and physical and emotional control (Diamante Baker, 1991; Howard Reardon, 1986; Johnson Kramer, 1961; Liggett Hamada, 1993; Scott, 1984), concentration (Collins, Doherty, Talbot, 1993; Schreiber, 1991), self-concept (Howard Reardon, 1986), motivation (Baybrooke, 1988; Darden, 1972; Pearson, 1983), and mood enhancement (Oakley, Norell, Cripps, 1995). Hypnosis has also been used to achieve optimal levels of performance arousal, which are necessary for the individual to achieve peak performance (Garver, 1977; Liggett, 2000; McMaster, 1993). Sports hypnosis has been further investigated in the literature in order to better understand the relationships among trance states, performance, pain, and healing. Researchers have examined occurrences of trance states in athletes and the effects of these states on performance and health (Callen, 1983; Cancio, 1991; Grove Lewis, 1996; Masters, 1992). Finally, in sports medicine, hypnosis has been utilized to decrease pain and increase healing (Ballinger, 1987; Kroger, 1988). Although performance is a primary concern for athletes, a variety of other issues exist which affect the mental health and functioning of athletes. Various developmental issues impact athletes, including identity issues, personal competence, and career planning (Jordan Denson, 1990; Pearson Petipas, 1990; Wooten, 1994). Athletes must also confront identity issues related to their level of participation and achievement in sports (Greendorfor Blinde, 1985; Ogilivie Howe, 1982; Svoboda Vanek, 1982). For athletes who focus solely on sports activities, self-esteem, identity, and social affiliation may be entirely dependent upon their athletic involvement (Gordett, 1985; Pearson Petipas, 1990). Once participation in athletics ceases (either voluntarily or involuntarily), the individual may experience crises related to identity, self-efficacy, and helplessness (Kleiber Brock, 1992). Athletes transitioning out of sports often must deal with rebuilding identity in a non-athletic context, constructing career-life plans, and developing personal competencies (Wooten Hinkle, 1991). These examples demonstrate the broad spectrum of developmental and identity issues that exist in the world of the athlete. However, the use of hypnosis in dealing with athletes developmental and identity issues has been conspicuously absent from the existing literature. Almost all descriptions of hypnosis in sports counseling are fundamentally connected to performance enhancement.

ABSTRACT The article examines the degree of institutionalization of the Guatemalan party universe across four areas: the pattern of interparty competition; the rootedness of parties in society; the legitimacy accorded to parties and democratic institutions; and the nature of internal party organization. Guatemala displays an extremely inchoate party structure across all these variables. There is no stability in the identity of the main parties in the polity. After more than two decades of electoral democracy, no single party has been able to avert a drift into electoral irrelevance or outright disappearance. With respect to the basic facets of internal party organization, Guatemalan parties exhibit a feebleness so pronounced that their very status as parties is questionable. In general, Guatemalan “parties” only fulfill Sartoris minimalist definition as organizations that field candidates for public office, but offer nothing more substantive. The obstacles to building a well-functioning liberal democracy in Guatemala are many and often acknowledged: the legacy of an overbearing military stemming from the “counterinsurgency state,” an inordinate degree of income inequality and poverty, and a large indigenous population that is fundamentally absent from political life, among others (Jonas 1991; Poitevin 1999). But the legacy of the countrys wretched past also includes another crucial element that often goes unmentioned: the peculiar nature of its party universe and its parties. This essay qualitatively assesses the degree of institutionalization of the Guatemalan party system. The variables used for this task are those conceptualized by Mainwaring and Scully (1995) in their pioneering work: the pattern of interparty competition, the rootedness of parties in society, the legitimacy accorded to parties and democratic institutions, and the nature of internal party organization. While these components of institutionalization can surely be correlated with one another, empirically speaking, party systems do not necessarily exhibit uniformity across these variables; that is, they may be hybrid and underinstitutionalized in some respects while adequately institutionalized in others. This article will show that the Guatemalan party universe is exceptionally inchoate across all the aforementioned four elements. Comparative studies of Latin American party systems have usually ignored Guatemala, not least because of the marginal role that parties in this country play in the political process. This study seeks to fill this gap and provide a case study of underinstitutionalization virtually without parallel in the region.

iHealth, an online suite of medical
practice web sites and secure patient email services provided by Medem is
now integrated with Google Health, a new product that allows users to
store, organize and manage their medical records and personal health
information online. Medem is a healthcare IT company founded by leading
medical societies, including the American Medical Association (AMA).
The iHealth collaboration with Google allows Google Health users to share
information in their Google Health profile with doctors using the iHealth
web product. This in turn helps keep doctors up-to-date on current medical
conditions and medications. Physicians who offer the iHealth service to
their patients will be listed in the Google Health “Find a Doctor” feature
as integrated with Google Health.
“Improving communication and strengthening the patient-physician bond is a
key goal for Medem and its participating medical societies,” explained

Dr. Rodney P. Steffen had been a dentist for four years in the U.S. Air Force when he opened a private dental practice in 1987 in Oklahoma City. He had a practice by himself until Dr. Kathy DeHart joined the practice in 1990. Steffen knew it made economic sense for more than one dentist to be operating out of one building. “Two dentists can work in a building and pay the same rent as one dentist,” he said. So Steffen put that knowledge to use in his practice.
